Young people today are involved in a variety of activities outside of the typical school day. These activities take place at school (e.g., choir, sports, journalism, and others) and others take place in the community (e.g., 4-H, sports, faith-based activities, and others). We would like to know about the reasons you are involved in activities. (This is a voluntary research project affiliated with Penn State and the University of Arizona. If you have questions please call the Penn State researcher coordinating the project. His name is Dr. Perkins and he can be reached at 814-863-1688.)

To fill out the survey, use your mouse to move to each question and click on the choices or type in your responses. Do not use the Enter key because it will jump you to the next screen before you are ready. Click on the submit buttons with your mouse when you are done with each screen.
